Honorarium Acknowledgment

By accepting the Honorarium, the Recipient acknowledges and agrees:

  • Honorariums are taxable income to Recipient. To the extent required, IEEE will report this income to the IRS and Recipient on Form 1099 (or 1042 for non-US recipients);
  • Recipient will provide IEEE a completed Form W-9 (US persons) or Form W-8BEN (non-US persons);
  • The Honorarium does not constitute compensation commensurate with the services provided;
  • IEEE  may publicize Recipient’s receipt of the Honorarium via any available media outlet.  Such publications may include, but are not limited to the inclusion of, the Recipient’s name, photograph and likeness;
  • The Recipient will comply with all laws and regulations that apply to its obligations and duties under this honorarium acknowledgement form, including, but not limited to, local laws in the area or country where the Honorarium will be used; 
  • The Recipient confirms that nothing herein shall be deemed or construed to create a joint venture, partnership, agency, or employer/employee relationship between the Recipient and the IEEE for any purpose, including, but not limited to, the payment of taxes or employee benefits.

Awards Acknowledgment

By accepting the Award, the Recipient acknowledges and agrees:

  • The Recipient shall comply with any reasonable requirements or restrictions specified by the Governing Body.
  • IEEE  may publicize Recipient’s receipt of the Award via any available media outlet.  Such publications may include, but are not limited to the inclusion of, the Recipient’s name, photograph and likeness.
